District Verification Form

As part of the McKay Scholarship payment process, the School Choice office sends a list of students scheduled to receive payment to the public school districts. District personnel indicate any of those students who are still enrolled in the public school system.

Payments are not generated for students enrolled in public school, and "EPS" is entered as the reason for non-payment on the student's payment tab. In the case that the district's report is incorrect, use a District Verification Form to request a correction.

The student's parent or guardian must complete the top portion of the District Verification Form, the school district McKay Scholarship contact must complete the middle section, and the private school representative should complete the bottom portion of the form.

Once the District Verification Form is received by our office, it is reviewed to determine if the student is eligible for the payment. If eligible, the missed payment may be received with the next batch of payments.

Scholarship Issue Form

The Scholarship Issue Form is used for a variety of issues related to McKay Scholarship students. Indicate the reason for submitting the form by checking one of the boxes near the top. If none of the options applies, use the "other" box, and briefly explain the reason. When requesting a missed payment, please indicate which payment or payments documentation is being submitted for.

Examples of documentation required for various issues are listed on the second page of the form. In most cases, once the form and documentation is received the issue can be resolved. However, further documentation may be requested. Please work with your regional manager to ensure all necessary documentation is received.

Guardian Issue Form

If a household needs to change the parent or guardian who is listed on the scholarship checks, the private school should submit a Guardian Issue Form and attach the applicable supporting documentation, such as the adoption documentation, new social security number, marriage/death certificate, or power of attorney documentation. A Parent Affidavit for the new guardian should also be submitted.

Parent Affidavit

According to the McKay Scholarship Program Rule (6A-6.0970), prior to receiving a scholarship payment, all parents of participating students must have on file with the Department Form IEPC-AFF1, Affidavit, signed and notarized affirming the validity of the parent's signature. The affidavits can be mailed or faxed to the School Choice office.

When a student begins participating in the McKay Scholarship Program or transfers to another private school while receiving the scholarship, the parent should complete the affidavit. If the guardian of a participating student changes, the new guardian should also file a McKay Scholarship Parent Affidavit Form.
