Slide 2 McKay Scholarship Program Rule requires that prior to receiving a scholarship payment, all parents of participating students must have on file with the Department an Affidavit, confirming the validity of the parent’s signature. Slide 3 Many schools ask parents to complete the Affidavit as part of their enrollment package. The parent or guardian whose name appears on scholarship payments must complete the affidavit, and a notary must witness the parent’s signature and document the parent’s identification. Slide 4 A Parent Affidavit must be submitted for each student new to the McKay Scholarship Program. When a student transfers to your school from another private school while receiving the scholarship, please ask the parent to complete an Affidavit at that time. In addition, when you submit a Guardian Issue Form to change the guardian’s name, you should also submit an affidavit confirming the new parent/guardian signature. Slide 5 Before submitting Affidavits to the School Choice office, please check to make sure each affidavit has been completed by the correct parent or guardian. The parent name and signature on the affidavit must match the name that appears on the scholarship payment. Also, please check to see that the notary portion of the affidavit has been filled out completely. Incorrect or incomplete affidavits will not fulfill the scholarship program requirement.
