Transcript for: “Scholarship Compliance Form” video. Slide 2 The Scholarship Compliance Form is available from your school’s Welcome Page on our website. Following is a brief overview of the contents of the Form and information about resources to help you understand compliance requirements and to complete the Form correctly. Slide 3 The Compliance Form serves as an application to participate in the McKay or Florida Tax Credit Scholarship programs, and participating private schools must certify ongoing compliance with scholarship requirements by submitting the form annually. Slide 4 The Form is divided into sections, and each is completed separately. Red “x’s” indicate questions that have not yet been answered. Click the header for the section you wish to complete. Slide 5 Selecting the header opens questions and answers for the section. Once you’ve selected your answers, click “continue” to return to the main form. Slide 6 In some cases, certain answers lead to additional questions. Select the section header again to complete, until no further red “x’s” appear. Slide 7 As you complete the Form, be sure to read the questions carefully. For example, in Section 3, question C relates to a requirement of the Florida Tax Credit Scholarship program. A “No” answer to this question will not meet compliance requirements for that program. Slide 8 Some sections include questions about your school’s policy #s, such as Worker’s Compensation and automobile insurance. You may wish to review the entire form first, and gather the information you will need to complete it. Slide 9 Many questions are taken directly from scholarship program statutes. The “Compliance Resources” section of our website provides additional information on these topics. Slide 10 Florida private schools are required to maintain health and immunization records for all students. See the “Detailed Requirements for Private Schools” area on our website for more information on this section and for links to Department of Health forms and procedures for students. Slide 11 The last section covers the requirements for your school’s physical location. Links to the Department of Health website and to DOH forms are available within the Compliance Form itself, and more information is available in the “Compliance Resources” area of our website. Slide 12 After submitting the form online, it must be printed, signed by the school’s director or principal, and notarized before being mailed to our office. The Scholarship Compliance Form is a sworn statement; please review it carefully before signing. Slide 13 Your school’s application to participate in the state scholarship programs is opened each year when your Regional Manager receives the hard copy Compliance Form in our office. Deadlines apply for schools renewing participation; refer to the Application/Renewal Instruction packet linked from our website.
